What are the dosing forms of bisphosphonates?

  • Alendronate is prescribed at 5 mg po daily or 35 mg po weekly for osteoporosis prevention, or 10 mg daily or 70 mg weekly for osteoporosis treatment.
  • Risedronate is prescribed at 5 mg po daily or 35 mg po weekly for prevention and treatment
  • Ibandronate is prescribed at 2.5 mg po daily or 150 mg po monthly for prevention and treatment. Ibandronate 3 mg IV every 3 months is another treatment option.
  • Zoledronic acid is prescribed 5 mg IV once yearly for treatment.

Bisphosphonates must be taken after a prolonged fast, usually in the morning before breakfast, with 8 ounces of water (no other liquid), 30 minutes (60 minutes for ibandronate) before eating or drinking. Patients should also remain upright for the 30 minutes (60 minutes for ibandronate) after taking the therapy.
